Community Nursing Care

At Reignite Support, we understand that nursing care is an essential part of managing health and wellbeing of participants who have high care needs. That’s why we offer Community Nursing Care services as part of our NDIS support services in Adelaide and surrounding areas.

Our Community Nursing Care services are designed to provide individuals with disabilities with the support they need to manage their health and wellbeing in the comfort of their own home. We work closely with our participants and their families to develop individualised care plans that meet their specific needs.

Some of the supports that our Community Nursing can offer are:

• Developing Health Care Plans [HCP]
• Continence advice and management
• Catheter care and management
• Tube-feeding assistance
• Wound care
• Stoma care
• Respiratory support
• Medication administration/supervision
• Dementia management
• Monitoring – BGLs, blood pressure and temperature.
• Diabetes management
